29 CFR 1910.101(b):The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tank cars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6: Employees were exposed to injury from fire and explosion or a projectile hazards. Employees in the Python Area were exposed to fire and explosion or projectile hazards in that compressed gas cylinder, that was not in use, was not secured to prevent damage in accordance with Compressed Gas Association Pamphlet P-1-1965. Propane gas cylinder located in the Python Area was stored unsecured.

29 CFR 1910.178(l)(2)(ii):The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace. Employees were exposed to struck-by hazards. Employees working at the Facility's Shop and Python Area were exposed to struck-by hazards. Employee was permitted to operate the forklift without being provided with a formal training. The employee was only provided with on the job practical training.
