(a) Perishable Warehouse- 400 Paintersville Road, New Stanton, PA: On or about February 21, 2022, employees were not provided with a safe means of access and egress while walking through the same roll-up doors the powered industrial trucks were using, with a limited clear path of travel for both parties.

29 CFR 1910.178(n)(6):Industrial truck drivers were not required to look in the direction of, and keep a clear view of the path of travel: (a) Perishable Warehouse, 400 Painterville Road, New Stanton, PA: On or about February 21, 2022, employees operating powered industrial trucks (PIT) did not have a clear view of their path of travel while traveling through the solid blue colored roll-up doors. An employee sustained injuries when they were struck-by a double pallet jack traveling through one of these areas.
