29 CFR  1926.416(a)(1):No employer shall permit an employee to work in such proximity to any part of an electric power circuit that the employee could contact the electric power circuit in the course of work, unless the employee is protected against electric shock by deenergizing the circuit and grounding it or by guarding it effectively by insulation or other means.  On or about March 1, 2022, employees were observed installing windows from an aerial lift located at, 3619 Bailey Ave, Cleveland, Ohio 44113; the employer did not ensure the employees were protected against electric shock by deenergizing the circuit and grounding it or by guarding it effectively by insulation or other means while maneuvering the aerial lift between the service power line and communication lines attached to power pole number 201102.

29 CFR  1926.453(b)(2)(v):A safety harness with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  On or about March 1, 2022, employees were observed installing windows from an aerial lift located at, 3619 Bailey Ave, Cleveland, Ohio 44113; the employer did not ensure the employees were protected from a fall that was approximately 25 feet above the landing surface by the use of safety harnesses with lanyards attached to the boom or basket.

On or about March 1, 2022, an employee was observed installing windows from an opening on the third level of a commercial building, without a means of fall protection located at, 3619 Bailey Ave, Cleveland, Ohio 44113; the employer did not ensure the employee was protected from a fall that was approximately 25 feet above the landing surface.
