The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  The training shall include the correct procedures for dealing with electrical hazards and for erecting, maintaining, and disassembling the fall protection systems and falling object protection systems being used.  On or about March 2, 2022, at a construction site located at 231 Achievement Drive in Brillion, Wisconsin, the employer did not provide required scaffold use safety training for employees working from scaffolding during siding installation on a two-story apartment building.

On or about March 2, 2022, at a construction site located at 231 Achievement Drive in Brillion, Wisconsin, an employee was exposed to a potential fall hazard of 17.8 feet while working from a 22-foot long aluminum platform installed with one end extended 9 feet past its pole support.

29 CFR  1926.1053(b)(1):  When portable ladders are used for access to an upper landing surface, the ladder side-rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder.   a)  On or about March 2, 2022, at a construction site located at 231 Achievement Drive in Brillion, Wisconsin, an employee was exposed to a potential fall hazard of 8 feet to a lower surface while using a roof access/egress ladder positioned with its top on the eave fascia board below the roof edge, ladder not secured, and no grabrails installed above the roof edge.  b)  On or about March 2, 2022, at a construction site located at 231 Achievement Drive in Brillion, Wisconsin, an employee was exposed to a potential fall hazard of 8 feet to a lower surface while using a roof access/egress ladder without the side-rails extended at least 3 feet above the roof edge.
