29 CFR  1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) BRILLIANT ELECTRIC SIGN CO., LTD. located at 515 Union Ave # 187, Dover, OH 44622:  On and preceding March 3, 2022, the employer did not ensure that employees wore head protection while working in an area with overhead hazard.  A crew was removing and lifting a sign using a truck mounted crane which was swung overhead thereby exposing them to struck-by hazards to the head.

29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   a) BRILLIANT ELECTRIC SIGN CO., LTD. located at 515 Union Ave # 187, Dover, OH 44622:  On and preceding March 3, 2022, the employer did not ensure that an employee working on the roof of a commercial building at heights exceeding 6 feet wore fall protection.  A sign removal crew member was working on a sloped roof of a commercial building unbolting and rigging the sign to prepare for the crane to lift a section of the sign to ground level.  The worker on the roof was not wearing  any form of fall protection thereby exposing the employee to falls in excess of 12 feet at the eave of the roof.

A crew was removing and lifting a sign using a truck mounted crane which was swung overhead without a tag line thereby exposing employees to struck-by hazards.
