29 CFR 1910.27(a): The employer did not ensure that scaffolds used in general industry met the requirements in 29 CFR part 1926, subpart L (Scaffolds) in the following instances.   (a) 1926.451(b)(1) Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports. On or about January 03, 2024, during an electrical sign repair project on a commercial building located at 16761 SouthPark Center, Strongsville, OH 44136, The employer did not ensure that each platform on all working levels of the supported scaffolding were fully planked and/or decked, exposing an employee to fall hazards.   (b) 1926.451(f)(3): Scaffolds and scaffold components were not inspected by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity. On or about January 03, 2024, , during an electrical sign repair project on a commercial building located at 16761 SouthPark Center, Strongsville, OH 44136, the employer did not ensure that the supported scaffolding was inspected prior to use, exposing employees to fall hazards due to deficiencies in the construction and installation of the scaffold.   (c) 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. On or about January 03, 2024, during an electrical sign repair project on a commercial building located at 16761 SouthPark Center, Strongsville, OH 44136, the employer did not ensure an employee working from a supported scaffold was protected from a fall of approximately (greater than 15 feet) by the use of a personal fall arrest system or guardrail system.
