Located at 3660 Kassidy Drive in Grove City, Ohio: On or about March 4, 2022, employees were sheathing the second story roof trusses while attached to unapproved rope lifelines that were tied in a knot at each attachment point, thereby exposing employees to an approximately twenty-foot fall hazard.

29 CFR  1926.502(d)(15):Anchorages used for attachment of personal fall arrest equipment shall be independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached.  a. Located at 3660 Kassidy Drive in Grove City, Ohio: On or about March 4, 2022, employees were sheathing the second story roof trusses with the rope lifelines tied around a single ridge anchor. The anchorage device was not properly secured to the roof's surface and was only rated for one employee attached, thereby exposing employees to an approximately twenty-foot fall hazard.

Located at 3660 Kassidy Drive in Grove City, Ohio: On or about March 4, 2022, the employer did not maintain an effective safety and health program to ensure that hazards were identified and corrected onsite while employees performed framing work.   Anthony Granite Construction was previously cited for a serious violation of this Occupational Safety and Health standard 29 CFR 1926.20(b)(1), which was contained in OSHA inspection number 1301852, citation number 1, item number 1a and was affirmed as a final order on 04/04/2018, with respect to a workplace located at 723 Viola Drive, Sunbury, Ohio 43074.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.   a. Located at 3660 Kassidy Drive in Grove City, Ohio: On or about March 4, 2022, employees were performing framing work and installing house wrap on the exterior wall without fall protection, thereby exposing employees to a fall hazard from approximately thirteen feet.   Jorge Ulloa Hernandez was previously cited for a serious violation of this Occupational Safety and Health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1489275, citation number 1, item number 1 and was affirmed as a final order on 10/14/2020, with respect to a workplace located at 8324 Comstock Terrace, Pickerington, Ohio 43147. 29 CFR  1926.1053(b)(1):When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support.  a. Located at 3660 Kassidy Drive in Grove City, Ohio: On or about March 4, 2022, employees were climbing from the top of two fiberglass step ladders in order to access the webbing of the roof trusses, thereby exposing employees to approximately nine foot fall hazards.
