29 CFR  1910.151(c):  Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use.  Reference: ANSI Z358.1 Emergency Eyewash and Shower Standard 5.4.4 The eyewash unit shall be located on the same level as the hazard and the path of travel shall be free of obstructions that may inhibit the immediate use of the equipment.  Building 24760:  In the following locations where corrosive chemical were used and handled, eyewash stations were obstructed and/or not available for immediate use:  a) Between administrative space and registry cage at custodial mixing station:  There was no emergency eye wash station in close proximity.

The employer did not require and enforce the use of eye protection by employees while using concentrated corrosive chemicals.  Employees were exposed to eye burns and blindness.

29 CFR  1910.1200(f)(10):  The employer did not ensure that workplace labels or other forms of warnings were prominently displayed on the container or readily available in the work area throughout each work shift:  Building 24760: Employees used unlabeled secondary containers to hold chemicals such as Portionpac Germicidal Cleaners 264N and  201N used during the facility cleaning process.  The employer did not ensure containers had prominently displayed labels showing chemical name, associated hazards, and warnings.  The employees were exposed to dermal burns or severe skin irritation.

29 CFR  1910.1200(h)(1):  Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  Building 24760:  Employees conducted facility cleaning, maintenance operations, and hand sanitizing using various chemicals such as Purell hand sanitizer, Portionpac Germicidal 264N and 201N, and WD40.  The employer did not provide effective information on the safety and health hazards specific to the chemicals assigned at the time of the employee's initial assignment or when a new hazard was introduced into the workplace.   Employees were exposed to fire hazards, severe dermal irritations, dermatitis, eye burns or blindness.
