29 CFR 1910.23(b)(9): The employer did not ensure that ladders  are inspected before initial use in each work shift, and more frequently as necessary, to identify any visible defects that could cause employee injury.  (a) North mall area; On or about the beginning of February 2022, a 16 foot, Louisville brand twin stepladder was not inspected for damage/defects before use.  Steps on the ladder were bent and cracked.

29 CFR 1910.23(c)(3): The employer did not ensure that ladders are not loaded beyond the maximum intended load.  Note to paragraph (c)(3): The maximum intended load, as defined in � 1910.21(b), includes the total load (weight and force) of the employee and all tools, equipment, and materials being carried.   (a) North mall area; On or about the beginning of February 2022, a 16 foot, Louisville brand twin stepladder was overloaded while two employees worked from the same ladder to replace track light fixtures.

29 CFR  1910.332(b)(1): Employees were not trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertained to their respective job assignments:  (a) Throughout the facility; On or about January 16, 2022 and the beginning of February 2022, employees were not trained how to distinguish exposed live parts from other parts of electrical equipment, how to determine voltage of exposed live parts, and the clearance distances before replacing track light fixtures on energized track rails.

29 CFR  1910.333(c)(2): Persons who were not qualified worked on electrical circuit parts or equipment that had not been de-energized under the procedures of paragraph (b) of this section. Such persons were not capable of working safely on energized circuits and were not familiar with the proper use of special precautionary techniques, personal protective equipment, insulating and shielding materials and insulated tools.  (a) Throughout the facility; On or about January 16, 2022 and the beginning of February 2022, unqualified employees replaced track light fixtures while the track rails were energized.
