29 CFR 1926.453(b)(2)(v):A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift. Note to paragraph (b)(2)(v): As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e). Employees were exposed to a fall hazard. Two employees were observed working from aerial lifts approximately 20' above ground without having their harness lanyards attached.

29 CFR 1926.1053(b)(4):Ladder(s) were used for purposes other than the purpose for which they were designed: An employee was exposed to a fall hazard. The exposed employee was observed standing on the fourth rung of an A frame ladder in the closed position while taking measurements. The ladder is intended to be opened when in use and the ladder was in the closed position. The balcony deck where the ladder was being used was 23' 4" above ground located at the job site, 1135 Technology Drive, O'Fallon, MO.

29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): Employees were exposed to fall hazards. Employees conducting siding installation at a residential construction site were exposed to a fall of approximately 20 feet from the apartment balcony to the ground. The employees were not using a fall protection system. Jeena Services, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501 (b) (1), which was contained in OSHA inspection number 1283307, citation number 1, item number 2 and was affirmed as a final order on 05/21/2018, with respect to a workplace located at The Villages of Duluth, 3465 Duluth Hwy. 120, Duluth, Georgia.
