On or about March 4, 2022, the employer did not ensure procedures were utilized for the control of potentially hazardous electrical energy of the Ground Service Equipment Shop number 3 roll up style garage door, while employees were engaged in service and maintenance activities, such as but not limited to, repair of the garage door limit device.

29 CFR 1910.147(d):Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: On or about March 4, 2022, the employer did not ensure employees performing service and maintenance activities on the Ground Service Equipment Shop number 3 roll up style garage door applied energy controls by; turning off or shutting down the machine [per 1910.147(d)(2)], isolating the electrical and hydraulic energy sources [(d)(3)], affixing lockout device(s) to the energy isolation devices [(d)(4)(i)], and verifying de-energization and isolation had been accomplished [(d)(6)], prior to repairing the garage door's limit control device to prevent unexpected energization of the chain and sprocket mechanism.

29 CFR 1910.147(c)(6)(i)(C): Where lockout was used for energy control, the periodic inspection did not include a review, between the inspector and each authorized employee, of that employee's responsibilities under the energy control procedure being inspected. On or about March 4, 2022, the employer did not ensure a periodic inspection, consisting of a review between an inspector and each authorized employee, of the authorized employee's responsibilities where lockout was required to be used for service and maintenance activities. A periodic inspection and/or lockout training was last conducted on Maintenance Technician employees who perform service and maintenance activities on machines and equipment at the facility, to include but not limited to, repairing electrically powered overhead garage doors, in 2020.
