29 CFR 1910.22(d)(2): The employer did not ensure hazardous conditions on walking-working surfaces were corrected or repaired before an employee used the walking-working surface again. When the correction or repair cannot be made immediately, the hazard was not guarded to prevent employees from using the walking-working surface until the hazard is corrected or repaired: (a) On or about March 1, 2022, employees working at the entrance of the car wash stood on a piece of metal which covers a floor opening next to the yellow lever that activates wheel holders that move the cars into the car wash. The piece of metal was not secured and fell numerous times exposing employees to a trip hazard of the floor hole.
