29 CFR 1926.1425(a): 29 CFR 1926.1425(a): The employer did not use available hoisting routes to minimize the exposure of employees to hoisted loads. (a) Jobsite located at 1 Dock St, Philadelphia PA: The employer failed to establish a safe hoist path during site hoisting operations in that employees not engaged in the hooking, unhooking, or guiding of a load (a two-leg wire rope bridle sling weighing approx. 99 pounds) were allowed to work within the fall zone and underneath a load being hoisted directly vertical, to an approximate height of 270 feet, while attached to the lifting hook of a Potain Hammerhead tower crane. The load disengaged from the crane hook, fell to the ground below and seriously injured two employees. Condition noted on or about the two-month period of January 5 - March 5, 2022.
