(a) On or about March 11, 2022, at the worksite located at 450 N. Main St., West Bend, WI, the employer did not ensure fall protection was used for personnel exposed to a fall hazard. A worker was removing metal ties from the top of a newly erected concrete wall and was exposed to a 13 foot fall hazard.
