29 CFR  1910.132(d)(1)(i):The employer did not select and have each affected employee use, the types of personal protective equipment that would protect the affected employee(s) from the hazards identified in the hazard assessment:  Site: Production floor, near the Blue Lane,  spray glue application:            Production floor, on orange and Blue Lanes, cleaning operation using Lacquer thinner.  a) On or about March 15, 2022, the employer did not provide chemical resistant glove to protect the workers from skin contact hazard during contact adhesive spraying operation.  b) On or about March 15, 2022, the employer did not provide chemical resistant gloves to protect the worker's hands during the cleaning operation. The workers were using lacquer thinner (flammable liquid category 2) chemical to wipe down finished wood products without wearing protective gloves.  c) On or about March 15, 2022 the employer did not provide face protection where the workers were engaged in spray glue application. ES80 Contact Adhesive (glue) was classified as skin contact hazard.  d) On or about March 15, 2022, the employer did not provide protective garment covering where the worker were engaged  in spray glue application.

29 CFR  1910.1001(j)(3)(i):Building and facility owners (the employer) did not determine the presence, location, and quantity of ACM and/or PACM at the work site. Employers and building and facility owners shall exercise due diligence in complying with these requirements to inform employers and employees about the presence and location of ACM and PACM.  a) On or about March 15, 2002, in Building 321, in the warehouse, where asbestos containing material (pipe insulation) was struck by forklifts rendering the material friable, the employer did not determine that the material was asbestos containing. Bulk sample taken during this inspection revealed that the material contained 30 per cent amosite asbestos.  b) On or bout March 15, 2022, The employer did not notify the workers of the presence of Asbestos Containing Material (ACM) or Presume Asbestos Containing Material (PACM) was present in the form of pipe insulation in the warehouse. The material became friable when struck by fork truck.

29 CFR  1910.1200(h)(2):The employer did not provide employees with effective information on hazardous chemicals in their work area at the time of their initial assignment and whenever a new chemical hazard the employees have not previously been trained about was introduced into their work area. Employees were not informed of the following: the requirements of this section; any operations in their work area where hazardous chemicals are present; and, the location and availability of the written hazard communication program, including the required list(s) of hazardous chemicals, and safety data sheets as required by this section.  Production floor, at Orange, Blue and Red Lanes:  a) On or about March 15, 2022, where employees worked with chemicals such as but not limited to Lacquer thinner, ES80 Contact Adhesive (spray glue on daily basis, the workers were not provided training on the hazard of the chemicals, the means to protect themselves,  the presence of a written hazard communication program and list and location of the safety data sheet of the chemicals prior to their job assignment.  b) On or about March 15, 2022, where workers used lacquer thinner to clean finished wood products on daily basis the employer did not provide training to the workers the hazards of the chemical and the means to protect themselves. Workers were observed using the chemical without any protective gloves.

29 CFR 1910.107(g)(1): Spraying was conducted outside of predetermined spraying areas:  On or about March 15, 2022, near the Blue Lane of the production floor spray application of E80 glue was conducted outside of a designated area.  At the time of the inspection, employees were spraying flammable EverStrong ES30 contact adhesive outside of paint booths. The booth was not operational.  PROCK OPERATIONS, INC. dba: FYC-FOR YOUR CONVINIENCE was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.107(g)(1), which was contained in OSHA inspection number 1376794, citation number 1, item number 1 and was affirmed as a final order on May 30, 2019, with respect to a workplace located at 321  East Hardy Street, Saint James, MO 65559.
