29 CFR 1926.25(a): Throughout this job site, during the course of construction operations, framing materials with nails, pipe and trash were lying in and adjacent to walkways and work areas, exposing employees to trip and fall hazards. Location - 303 Woburn Street, Lexington, MA. On or about March 17, 2022, five employees were exposed to debris, trash, protruding nails that had not been removed from the work area. These hazards existed from the interior of the garage and extended out to the front of the house and abutted against bundles of new wood.

29 CFR 1926.501(b)(13): Employees engaged in residential construction activities 6 feet (1.8m) or more above the lower levels were not protected by guardrail systems, safety net systems, or personal fall arrest systems. Location - 303 Woburn Street, Lexington, MA. Instance A: On or about March 17, 2022, five employees were exposed to fall hazards greater than fifteen feet above lower level from multiple window openings. Instance B: Three employees were working/walking next to unprotected side/edges at multiple locations on second floor deck. These areas include multiple outside edges greater than thirteen feet above lower level and multiple internal unprotected edges on second floor deck at heights ten feet above first floor deck. The employees were framing the house and created the unprotected edges but did not address the fall hazards. Repeat 1: Sox Construction Inc, DBA: Sox Framing Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13) which was contained in OSHA inspection number 1521798, citation number 1, item number1, and was affirmed as a final order on 6/08/2021, with respect to a workplace located at 7 Goreham Street, Chelmsford, MA 01824. Repeat 2: Sox Construction Inc, DBA: Sox Framing Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13) which was contained in OSHA inspection number 1495398, citation number 1, item number 3 and was affirmed as a final order on 12/29/2020, with respect to a workplace located at 7 Goreham Street, Chelmsford, MA 01824.
