29 CFR 1926.602(c)(1)(vi): Section 603(e) of American National Standard Institute, B56.1-1969 Safety Standards for Powered Industrial Trucks, as adopted by 29 CFR 1926.602(c)(1)(vi): When a truck was left unattended, load engaging means were not fully lowered, controls were not neutralized, power was not shut off, brakes were not set, and the key or connector plug was not removed. Location: Jobsite; On or about 10/31/2023, a Caterpillar (CAT) TH360B rough terrain forklift was left unattended with the load suspended at a 2nd story window of the residence that was in the vicinity of the front entrance of the building as well as a ladder which employees utilized to access the 2nd story balcony area.

Location: Jobsite; On or about 10/31/2023, an employee was not protected from falling off the peak of the roof truss they were working on which was greater than 25-feet above the ground below and was 19-feet above the floor of the 2nd story of the residence.
