On or about March 15, 2022 at jobsite 600 S. Berkley Ave., Elmhurst, IL 60126, the employer did not ensure that the employee using an aerial lift utilized fall protection. Employee was exposed to fall hazards approximately 15 feet while engaged in cutting tree branch on a basket.

29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee wear appropriate eye or face protection when exposed to eye or face hazards from flying particles: On or about March 15, 2022 at jobsite 600 S. Berkley Ave., Elmhurst, IL 60126, the employer did not ensure that the employees working on ground level were using eye protection. Employees were exposed to eye injuries when trimmed tree branches falling to ground and when using the wood chipper. Eye protection was not being utilized when engaged in these activities. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d)
