The employer permitted employees to climb up the platform of an exhaust fan system, approximately 7 feet above ground level, to clean out the saw dust buildup, without a guard rail system.

The following cords were damaged.  a) Next to the Edger Saw a power cable used to pump and power the ventilation system, the outer jacket was damaged and did not provide any strain relief. b) Next to the Planner, a power cable used to power the planner control box, the outer jacket was damaged and did not provide any strain relief.
