29 CFR 1910.95(d)(1):When information indicates that any employee's exposure may equal or exceed an 8-hour time-weighted average of 85 decibels, the employer shall develop and implement a monitoring program. On or about June 3, 2022, operators working on a one-man dismantler pallet machine were exposed to high noise levels equivalent or exceeding the 8-hour time weighted average (TWA) of 85 dBA and the employer did not ensure a noise monitoring program was implemented onsite for the following: a. On or about June 3, 2022, a breakdown operator was exposed to noise at 154.2% of the permissible daily dose, or an average sound level of 93.5 dBA, as measured over 456 minutes of sampling. This does is equivalent to an 8 hour TWA exposure of 93.1 dBA accounting for the period of 24 minutes unmonitored. b. On or about June 3, 2022, a #1 bandsaw operator was exposed to noise at 425.5% of the permissible daily dose, or an average sound level of of 100.4 dBA, as measured over 480 minutes of sampling. This dose is equivalent to an 8 hour TWA exposure of 100.4 accounting for the period of 0 minutes unmonitored. c. On or about June 3, 2022, a #2 bandsaw operator was exposed to noise at 505.6% of the permissible daily dose, or an average sound level of 101.6 dBA, as measured over 480 minutes of sampling. This dose is equivalent to an 8 hour TWA exposure of 101.7 dBA accounting for the period of 0 minutes unmonitored. d. On or about June 3, 2022, a smartsaw operator was exposed to noise at 209.4% of the permissible daily dose, or an average sound level of 95.5 dBA, as measured over 471 minutes of sampling. This does is equivalent to an 8 hour TWA exposure of 95.3 dBA accounting for the period of 9 minutes unmonitored. There is no abatement certification or documentation required for this item.

At least annually after obtaining the baseline audiogram, the employer shall obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels. On or about June 3, 2022, a bandsaw operator working on a one man dismantler pallet machine was exposed to noise at 425.5% of the permissible daily dose, or an average sound level of of 100.4 dBA, as measured over 480 minutes of sampling. This dose is equivalent to an 8 hour TWA exposure of 100.4 accounting for the period of 0 minutes unmonitored and the employer did not ensure annual audiograms were established for employees who noise levels were equivalent or exceeded the 8-hour time weighted average (TWA) of 85 dBA . Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.95(k)(1):The employer shall train each employee who is exposed to noise at or above an 8-hour time weighted average of 85 decibels in accordance with the requirements of this section. The employer shall institute a training program and ensure employee participation in the program. On or about June 3, 2022, operators working on a one-man dismantler pallet machine were exposed to high noise levels equivalent or exceeding the 8-hour time weighted average (TWA) of 85 dBA and the employer did not ensure training was provided on various elements of a hearing conservation program such as, but not limited to, the effects of noise, hearing protection, and audiometric testing for the following: a. On or about June 3, 2022, a #2 bandsaw operator was exposed to noise at 505.6% of the permissible daily dose, or an average sound level of 101.6 dBA, as measured over 480 minutes of sampling. This dose is equivalent to an 8 hour TWA exposure of 101.7 dBA accounting for the period of 0 minutes unmonitored. b. On or about June 3, 2022, a smartsaw operator was exposed to noise at 209.4% of the permissible daily dose, or an average sound level of 95.5 dBA, as measured over 471 minutes of sampling. This does is equivalent to an 8 hour TWA exposure of 95.3 dBA accounting for the period of 9 minutes unmonitored. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.212(a)(3)(ii):The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. On or about June 3, 2022, bandsaw operators were exposed to amputation hazards while working on a one-man dismantler pallet machine and the employer did not ensure adequate guarding was installed to eliminate hazards such as those created by the point of operation. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
