29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects:  A) 2783 Woodbridge Ave Edison, NJ 08817  Employee working on the ground level cleaning up debris was not protected from falling materials. The exposed employee could be subjected to head trauma from falling materials and tools from employees working above on roof.   Occurred on or about 03/22/22

29 CFR 1926.503(b)(1):The employer did not verify compliance with paragraph (a) of this section (fall protection training) by preparing a written certification record which contained the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:  A) 2783 Woodbridge Ave Edison, NJ 08817  The employer did not ensure, through a written certification, that employees had been adequately trained in the hazards associated with falls and the proper ways and procedures to take in order to minimize the hazards. 29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system or personal fall arrest systems:  A) 2783 Woodbridge Ave Edison, NJ 08817  Instance A  Three employees were exposed to fall hazards of up to 23' while performing residential roofing activities without any fall protection.  Instance B  Two employees were exposed to fall hazards of up to 12'5'' while performing residential roofing activities without any fall protection.   Occurred on or about 03/22/22  The DC Services LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number1532180, citation number 1, item number 1 and was affirmed as a final order on 12/15/21, with respect to a workplace located at 4 Sandra Drive, Edison, NJ 08820.
