29 CFR  1926.102(a)(1): Employees did not use eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  a) 331 Boulevard, New Milford, NJ (Exterior Front): Employees engaged in roofing work were using hand-held pneumatic nail guns without the use of eye and face protection. The employer did not ensure that employees were protected with eye and face protection, thus exposing employees to an eye injury, on or about 9/10/22.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) 331 Boulevard, New Milford (Exterior Front): Employees engaged in roofing work were exposed to a fall hazard of approximately 26 feet above the lower surface, while using an extension ladder that was not extended 3 feet above the roofs landing. The employer did not ensure that the ladder side rails extended at least 3 feet above the upper landing surface of the roof, thus exposing employees to a fall hazard, on or about 9/10/22.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a) 331 Boulevard, New Milford (Exterior Front): Employees engaged in roofing work were exposed to a fall hazard of approximately 26 feet above the lower surface, without the use of a personal fall arrest system or other means of fall protection. The employer did not ensure that employees performing roofing work were provided with a personal fall arrest system, or other means of fall protection, thus exposing employees to a fall hazard, on or about 9/10/22.  DC Services, LLC was previously cited for a repeat violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1584927, citation number 2, item number 1 and was affirmed as a final order on 06/15/22, with respect to a workplace located at 2783 Woodbridge Ave, Edison, NJ 08817.  DC Services, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1532180, citation number 1, item number 1 and was affirmed as a final order on 12/15/21, with respect to a workplace located at 4 Sandra Drive, Edison, NJ 08820.
