29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) Jobsite: On or about October 1, 2021, a subcontractor employee was working on the roof and he was wearing the proper type of harness but he was tied of at a point at his waist. The fall hazard ranged from about 10 to 20 feet. The pitch of the roof was about 7 in 12.

29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder: (a) Jobsite: On or about October 2, 2021, the contractor did not ensure that the subcontractor had ladders set up to minimize fall hazards.
