29 CFR 1910.134(f)(2): The employer did not ensure that an employee using a tight-fitting facepiece respirator was fit tested prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) is used, and at least annually thereafter. Department of Veterans Affairs DBA Cincinnati VA Medical Center located at 3200 Vine Street, Cincinnati, OH 45220: a) On or about March 22, 2022 the employer did not provide annual fit tests to all employees required to wear a respirators. The employer required Registered Nurses working on the 5 North inpatient floor to wear tight fitting N95 filtering facepiece respirators to protect against the SARS-CoV-2 virus while providing direct patient care to confirmed COVID-19 positive patients.
