29 CFR 1910.151(c): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use. Reference: ANSI Z358.1 Emergency Eyewash and Shower Standard. On or about March 23, 2022, employees working on the JenFab Washer were exposed to injurious corrosive materials and were not provided with suitable facilities for quick drenching or flushing of the eyes and body.

29 CFR 1910.303(g)(1)(iii): At least one entrance of sufficient area shall be provided to give access to the working space about electric equipment. On or about March 23, 2022, electrical disconnecting switches and switching devices were not provided with sufficient area to give access to the working space of electrical equipment. Specifically: a. The switching device (circuit breaker) panel labeled "Panel C" and "MSBC, Panel 1" located on the east wall, was not provided with sufficient area for accessing the working space. b. The electrical disconnecting switch labeled "Trimmer 405", was not provided with sufficient area for accessing the working space. c. The unlabeled switching device (circuit breaker) panel, located next to Rifle Loader 108, was not provided with sufficient area for accessing the working space.
