29 CFR 1910.178(p)(l): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s): On 3/21/2022 and times prior to, Reed Lumber, LLC did not ensure powered industrial vehicles were removed from service when found to be in need of repair, defective or in any way unsafe. The following instances were identified: a. Komatsu, Model # FG25T-14, Serial # 588416A, was missing alignment pin for liquified petroleum gas tank, the clutch pedal did not have a non-slip cover, vision was blocked by installing cardboard on the overhead guard. b. Komatsu, Model # FG25T-16, Serial # A224349, was missing alignment pin for the liquified petroleum gas tank. c. Komatsu, Model # FG40ZT-8, Serial # 132839A, was missing alignment pin for the liquified petroleum gas tank, broken left turn signal, and blocked vision by installing plywood on the overhead guard.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during the operating cycle(s): On 3/21/2022 and times prior to, employees of Reed Lumber, LLC were exposed to struck by, caught in hazards while operating a Precision Husky wood chipper, Serial # C2654 with a horizontal feed. The infeed chute measured approximately 15.5 inches from the opening to the blade face. An employee suffered an amputation while operating the machine on 3/15/2022.
