29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment: Maintenance Shop: On or about March 23, 2022, Electrical Panels were blocked in three areas, exposing employees to electrical hazards. Each electrical panel had signage to not block and to keep clear. a) A 240V electric control panel was blocked with items stored around it, to include broom, metal, and bucket. b) A 240V electrical panel had items stored below it, included a water hose, stool and electrical source. c) A Wash bay electrical panel had items stored in the work area such as boxes, pallet, mouse traps on plywood boards, and a battery.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure: a) Main Wall work bench: On or about March 23, 2022, Orange extension cord used for tools/chargers had been zip tied to the electrical conduit on the wall with zip ties. Observed used/plugged into the multi outlet flexible cord was a heat gun. b) Back wall above Tool Cabinet: On or about Mar 23, 2022, Yellow extension cord used for extension cord physically attached to the wall and a metal industrial storage rack using zip ties to supply power to wall mounted speaker system.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: a) Maintenance Shop Work bench: On or about March 23, 2022 a relocatable power tap was physically affixed to a workbench used to power lighting for the work area, exposing employees to fire hazards. Matson Navigation Company of Alaska was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.303(b)(2), which was contained in OSHA inspection number 1522784, citation number 1, item number 1 and was affirmed as a final order on September 27, 2021, with respect to a workplace located at Dutch Harbor, Alaska.
