On or about March 24, 2022 at 200 West Railroad Street, Columbiana, OH 44408, the employer failed to ensure employees utilized lockout/tagout procedures in that employees grease the forge welder daily and are exposed to pneumatic and electrical hazards and have not been audited.   b. On or about March 24, 2022 at 200 West Railroad Street, Columbiana, OH 44408, the employer failed to ensure employees utilized lockout/tagout procedures in that employees change the dies on the C Frame-Press and are exposed to electrical and hydraulic hazards and have not been audited.   c. On or about March 24, 2022 at 200 West Railroad Street, Columbiana, OH 44408, the employer failed to ensure employees utilized lockout/tagout procedures in that maintenance employees lock out equipment with electrical, pneumatic and hydraulic hazards and have not been audited.

On or about March 24, 2022 at 200 West Railroad Street, Columbiana, OH 44408, the employer failed to ensure the hook on the CM half-ton crane, model number F, serial number L 1501 F, was inspected and documented monthly in that deficiencies identified by a third party company were not repaired.
