OSH ACT of 1970 Section (5)(a)(1): of the Occupational Safety and Health Act: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to caught in hazards from guiding hot piping into the rollers. (a) Zekelman Industries, Inc., on or about March 25, 2022 at the address of 1 Council Street, Wheatland, PA: An employee was using a three foot guide bar to direct hot piping onto the rollers on the piping machine. The guide bar got caught between the piping and the rollers and pulling the individual into the hot piping, resulting in burns to the employees hand.

(a) Zekelman Industries, Inc., at the address of 1 Council Street, Wheatland, PA. On or about March 25, 2022 and times prior thereto: Full oxygen compressed gas storage area- Ten oxygen compressed gas cylinders were free standing and not secured. (b) Zekelman Industries, Inc., at the address of 1 Council Street, Wheatland, PA. On or about March 25, 2022 and times prior thereto: Empty oxygen compressed gas storage area- One oxygen compressed gas cylinder was observed in the storage area free standing and not secured. (c) Zekelman Industries, Inc., at the address of 1 Council Street, Wheatland, PA. On or about March 25, 2022 and times prior thereto: A brown and orange compressed gas cylinder was free standing and not secured.

29 CFR 1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6: Section 5.5 Valve protection caps and valve outlet caps and plugs: Valve protection caps for a full or partially full cylinder designed to accept a cap shall always be in place and hand-tight except when these cylinders are in process or connected for use: (a) Zekelman Industries, Inc., on or about March 25, 2022 at the address of 1 Council Street, Wheatland, PA: The full oxygen cylinder storage- The orange and brown cylinder was observed without a cylinder protection cap while not in use.

The primary identifier is the container label: A) Zekelman Industries, Inc., at the address of 1 Council Street, Wheatland, PA. On or about March 25, 2022 and times prior thereto: The label on orange/rust colored compressed gas cylinder was torn/defaced. The placard identification portion of the label on the compressed gas cylinder was not discernable.
