West facing walkway of the asphalt plant:  On or about March 23, 2022, and at times prior, employee's were exposed to fall hazards while walking and conducting repair operations on elevated platforms at heights above 10 feet to the ground below without fall protection. The guardrail system being utilized was missing midrails.    b. North facing walkway of asphalt plant:  On or about March 23, 2022, and at times prior, employees were exposed to fall hazards while working and walking on elevated platforms at heights above 10 feet to the ground below without fall protection. The guardrail system being utilized had midrails bound together with baling wire.   c.  Elevated platform near Dryer:  On or about March 23, and at times prior, employee's were exposed to fall hazards while working and walking on an elevated platform at heights of approximately 7-8 feet to the ground below without fall protection.
