29 CFR 1910.22(d)(1): Walking-working surfaces were not inspected, regularly and as necessary, and maintained in a safe condition: a) Warehouse Near Cooler #2 - On October 22, 2019, the employer did not ensure that the floor was maintained in a safe condition where material handling equipment was used. There were large cracks and grooves in the concrete floor making it uneven.
