29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee is fit tested or required to use the respirator in the workplace: In the facility at 66 Broad St, Westfield, MA 01085: On or about March 03, 2022, employees required to use N95 respirators during tasks, such as, but not limited to, the provision of medication, treatment, and other care as necessary to residents confirmed positive for COVID-19, were not medically evaluated for their ability to use a respirator. Governor's Center RE LLC. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.134(f)(2), which was contained in OSHA Inspection number 1486939, citation number 1 item number 1b and was affirmed as a final order on December 1, 2020, with respect to a workplace located at 66 Broad St. Westfield, MA 01085

29 CFR 1910.134(f)(2):The employer did not ensure that employees using a tight-fitting respirator were fit tested prior to initial use of the respirator, whenever a different respirator facepiece was used, and at least annually thereafter: Facility at 66 Broad St. Westfield, MA 01085: On and about March 03, 2022, the employer did not provide a fit test to all employees required to wear a respirator, prior to initial use of the respirator, when a different respirator was used and at least annually thereafter. Employees were required to wear N95 filtering-facepiece respirators to protect against SARS-CoV-2 virus while providing care, such care as feeding, washing, dressing, and other personal care, to suspected and confirmed COVID-19 positive residents. Governor's Center RE LLC. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.134(f)(2), which was contained in OSHA Inspection number 1486939, citation number 1 item number 1c and was affirmed as a final order on December 1, 2020, with respect to a workplace located at 66 Broad St. Westfield, MA 01085
