29 CFR 1926.652(a)(1):Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or (c). On or about March 25, 2022, one employee was in a trench at an area which was up to approximately 8 feet and 9 inches deep to level a manhole without the use of a protective system: exposing the employee to a cave-in hazard.
