On or about March 28, 2022, at 3405 South Orlando Drive, Sanford, FL, two employees were working at the bottom of a 12 foot deep trench where a local point dewatering system used to control ground water intrusion was not monitored by a competent person, and approximately two feet of water was accumulating in the trench, increasing the risk of a trench collapse. No abatement certification or documentation required for this item.

On or about March 28, 2022, at 3405 South Orlando Drive, Sanford, FL, two employees were working in the bottom of an approximately 12 feet deep trench with approximately 2 feet of water in the trench bottom, and cave in protection was not used. b. On or about June 10, 2022, at 180 Airport Road, Sanford, FL employees were working in the bottom of a trench approximately 6 feet deep, with water present from a broken water main, and cave in protection was not used. No abatement certification or documentation required for this item. Masci Construction Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.652(A)(1), which was contained in OSHA inspection number 1253602, citation number 1, item number 1 and was affirmed as a final order on August 6, 2018, with respect to a workplace located at the intersection of MaBette Road and Palm Ave, Kissimmee, FL 34745.
