29 CFR 1910.176(a):Aisle(s) and passageway(s) were not kept clear and in good repair with no obstruction across or in aisles that could create a hazard. a) Edge Plastic Inc. on or about March 21, 2022, at the address of 449 Newman St. Mansfield OH 44902: The employer did not ensure aisleways in which fork trucks travel were kept clear of other employees' pallet wrapping stations/locations. Employees tasked with pallet wrapping duties within the aisleway of forklift travel were exposed to struck-by hazards.

29 CFR 1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. a) Edge Plastic Inc. On or about March 21, 2022, at the address of 449 Newman St. Mansfield OH 44902: The employer failed to report on employee hospitalization (admittance) following a workplace incident (broken ankle). The employer or employee representative failed to contact the Toledo Area Office or the 24-hour OSHA number to notify the agency of the employee hospitalization within 24 hours of admittance. The injured employee was admitted on March 21, 2022, for related in-patient treatment. The employer did not report the incident to the Toledo Area Office until March 23, 2022.
