29 CFR  1926.20(b)(1):The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:  On or about March 29, 2022 the employer Jace Roofing and Construction, LLC at jobsite located at 5055 E. Lake Rd. Sheffield Lake OH 44054, did not have a plan or a program to identify job site hazards which assists employees in maintaining a safe working environment. Such programs would support the employer and employees in identifying and avoiding potential hazards such as falls and struck by hazards during job site operations, decreasing the probability of employee injury or illness.

Lake Rd. Sheffield Lake OH 44054, did not have a fall protection training program in place to enable each employee to recognize the hazards of falling, and procedures to minimize these hazards. Such programs would support the employer and employees in identifying and avoiding potential hazards such as falls during job site operations, decreasing the probability of employee injury or illness.

Sheffield Lake OH 44054, did not ensure employees were protected from falls while conducting residential roofing construction activities on the second and third level of a new construction build stick frame multi-family new construction home, while exposed to a fall of approximately 19 feet or greater to the ground below.

Lake Rd. Sheffield Lake OH 44054, did not ensure the side rails of one portable aluminum multi-function ladder was extended at least three feet above the landing surface. At least one employee was exposed to falls from approximately 8 feet from the top of the third level to the decking of the second level, and approximately 19 feet to the ground below while using the ladder to access the top of the third level to perform roofing activities on a new build residential multi-family townhome.

29 CFR  1926.1060(a):The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards.  On or about March 29, 2022 the employer Jace Roofing and Construction, LLC at jobsite located at 5055 E. Lake Rd. Sheffield Lake OH 44054, did not provide a training program for each employee potentially exposed to fall hazards while using a portable ladder to enable each employee to recognize the hazards of falling from the ladder and the procedures to be followed in order to minimize these hazards. Four employees using the aluminum extension ladder to access the roof to perform roofing work were exposed to fall hazards of approximately 19 feet to the ground below. At least one employee was exposed to falls from approximately 8 feet from the top of the third level to the decking of the second level, and approximately 19 feet to the ground below while using the portable multi-function ladder to access the top of the third level to perform roofing activities on a new build residential multi-family townhome.
