1910.29(b)(13)(i): The guardrail system opening used around holes that served as points of access (such as ladderways), did not have a self-closing gate that slides or swings away from the hole, and was equipped with a top rail and midrail or equivalent intermediate member that met the requirements in paragraph (b) of this section: On or about March 29, 2022, employees were exposed to fall hazards when climbing a fixed ladder above the drum roller because the ladderway was not equipped with a self-closing gate.
