29 CFR  1910.28(b)(3)(ii): The employer did not ensure that each employee was protected from tripping into or stepping into or through any hole that is less than 4 feet (1.2 m) above a lower level by covers or guardrail systems.    On or about November 01, 2024, in the Mixing area, a floor drain channel was uncovered across the floor area creating a trip hazard in the path of travel to the Motor Blend Mixer and to the emergency safety shower in case of chemical exposure.

29 CFR  1910.28(b)(1)(i): Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):    On or about November 01, 2024, at the Mixing area, three employees performing maintenance on the sulfuric acid line which was stationed above 4 feet from the lower surface worked on top of the Motor Blend Mixer without fall protection.  Westway Feed Products LLC was previously cited for a violation of an equivalent Occupational Safety and Health Standard, 1910.28(b)(6)(i), which was contained in OSHA inspection number 1586444, citation number 1, item number 1, and was affirmed as a final order on 05/18/2022, with respect to a workplace located at 22220 Route 84 North, Cordova, Illinois 61242.
