29 CFR 1926.100(a):Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns: On or about March 29, 2022, the employer Integrity Construction II at worksite located at 5055 E. Lake Rd. Sheffield Lake OH 44054, did not ensure that employees were protected from head injury hazards by protective hardhats while working directly below and under a Komatsu RDK 9699 excavator within a water line excavation. Employees were exposed to potential struck by injuries caused by being struck by the heavy equipment excavator arm and/or falling construction debris, soil and other materials.
