Integrity Construction II, Inc., at a jobsite located at 5524 Bachman Road, Canal Winchester, OH 43110: On or about January 10, 2025, the employer did not ensure a safe means of egress was available in an excavation measuring over six feet deep while an employee was working to install a manhole structure.

29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins, indications of failure of protective systems. a. Integity Construction II, Inc., at a jobsite located at 5524 Bachman Road, Canal Winchester, OH 43110: On or about January 10, 2025, the employer did not ensure daily inspections of excavations, the adjacent areas, and protective systems were made by a competent person. Employees were working in an excavation at depths of over six feet without safe access/egress and without a compliant protective system in place to prevent collapse. Inspections were not performed by a competent person to determine compliant protective measures needed to work in the excavation with these conditions. These conditions exposed employees to struck-by and caught-in hazards.

Integity Construction II, Inc., at a jobsite located at 5524 Bachman Road, Canal Winchester, OH 43110: On or about January 10, 2025, the employer did not ensure a compliant means of cave in protection was being used to protect employees while working in a trench of over six feet deep. Employees were exposed to cave-in hazards when accessing the trench boxes due to a nine foot long section of unprotected area from the dirt ramp employees were climbing down into the trench
