Employees  were working in an excavation at depths of over six feet without safe access/egress and without a compliant protective system in place to prevent collapse. Inspections were not performed by a competent person to determine compliant protective measures needed to work in the excavation with these conditions. These conditions exposed employees to struck-by and caught-in hazards.
