Archbold, OH: The employer did not ensure that energy isolating procedures for shutdown were performed prior to performing blade change on the HYD-MECH V-18 Band saw #369. Employees performing these tasks in energized condition were exposed to amputation hazards at or near the machine's tensioning wheel and  1 ?" blade, in that energy control shutdown procedures required under 29 CFR 1910.147(d)(2) were not utilized before the employee began the aforementioned work task on the machine.
