29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a) Haulotte North America Manufacturing. on or about March 21, 2022, at the address of 125 Taylor Parkway. Archbold, OH: The employer did not ensure that energy isolating procedures for shutdown were performed prior to performing blade change on the HYD-MECH V-18 Band saw #369. Employees performing these tasks in energized condition were exposed to amputation hazards at or near the machine's tensioning wheel and 1 ?" blade, in that energy control shutdown procedures required under 29 CFR 1910.147(d)(2) were not utilized before the employee began the aforementioned work task on the machine.

Employees operating the saw near the unguarded hydraulic clamps were exposed to crushing hazards.

29 CFR 1910.212(a)(3)(ii):Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) Haulotte North America Mfg. on or about March 24, 2022, at the address of 125 Taylor Parkway Archbold, OH: The employer did not ensure that the point of operation of the Hyd-Mech band saw #369 (Model: V-18; located in weld/fabrication department was adequately guarded. After the material was cut, the unguarded blade continued to travel towards the operator a minimum of 9" beyond the material being cut. In this condition employees were exposed to amputation hazards.
