29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section.  (a) On or about March 30, 2022, and April 12, 2022, the employer did not ensure that machine specific procedures for the control of hazardous energies (LO/TO) were utilized during service and maintenance of the headers #50 (A97-2), #44 (A97-1), #41 (A96-1), #54, and Jern Yao. Employees were performing tool changes without the use of LO/TO. Hazardous energies present on the equipment include, but are not limited to, pneumatic, hydraulic, electrical, and mechanical.    (b) On or about April 12, 2022, the employer did not ensure that machine specific procedures for the control of hazardous energies (LO/TO) were utilized during service and maintenance of the Weingarten mechanical forging press. An employee performed an adjustment to the forging press without the use of LO/TO. Hazardous energies present on the equipment include, but are not limited to, pneumatic, hydraulic, electrical, and mechanical.

Employees operate the Weingarten mechanical forging press which is equipped with two hand control devices for operation to prevent employees from reaching into the point of operation during hazardous cycle.

29 CFR  1910.303(b)(7)(i): Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings shall be effectively closed to afford protection substantially equivalent to the wall of the equipment.  a) On or about March 30, 2022, the employer did not ensure that #40 Waterbury Hand (K15-1) was free from recognized electrical hazards to prevent employees from electrical shock. Employees are exposed to a 120-volt electrical shock during the set up and maintenance of the machine. Specifically, the 6x6 control box exterior housing was not secure by ensuring all knockouts were closed.  b) On or about April 12, 2022, the employer did not ensure that Header # 41 (A 96- 1) was free from recognized electrical hazards to prevent employees from electrical shock. Employees are exposed to a 120-volt electrical shock during the set up and operation of the machine for production purposes. Specifically, the 4x4 dual receptacle located near the control panel was missing one receptacle exposing conductors to employees.

29 CFR  1910.303(g)(2)(i): Except as�elsewhere required�or permitted�by this�standard, live�parts of�electric equipment�operating at�50 volts�or more�shall be�guarded against�accidental contact by�use of�approved cabinets�or other�forms of�approved enclosures:  a) On or about March 30, 2022, the employer did not ensure that #K8 Roll Thread Machine (K27-8) (R-1) was guarded against accidental contact by use of an approved enclosure. Specifically, the 3-phase, 460-volt, 20.6-amp electrical control compartments, control wires passing through the knock outs on the exterior of the enclosure.  b) On or about April 12, 2022,  the employer did not ensure that #T 3 Roll Thread Machine (K27-7) was guarded against accidental contact by use of an approved enclosure. Specifically, the broken face plate on the 2x4 box, a 120-volt receptacle on the exterior of the machine, and the 1/2" EMT is separated from the box exposing conductors.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  a) On or about March 29, 2022, and July 14, 2022, the employer did not ensure that operators assigned to hand-feed materials into unguarded points-of-operation were protected from the pinch points and/or in-going nip points of moving and/or rotating parts likely to result in serious injury. Operators also had access to unguarded danger zones of moving mechanical parts such as, but not limited to die rams, drive shafts, universals, rockers, and pusher assemblies on: - Reed thread rollers, model B-112, (K27-3) (R1) and (K-27-4) (R3). - Waterbury thread rollers, models Hand #20 (K11-2) (A2); Hand #30 (K12-1) (A9), (K12-2) (A8), (K12-4) (A7); Hand #40 (K15-1) and (K15-3); and Hand #60 (K30-2); and, - Tesker thread rollers, T1 (K27-5), T2 (K27-6), T3 (K27-7), and 45-2.  b) On or about April 12, 2022 and July 14, 2022, the employer did not ensure that operators assigned to hand-feed materials into unguarded points-of-operation were protected from the pinch points and/or in-going nip points of moving and/or rotating parts likely to result in serious injury. Operators also had access to unguarded danger zones of moving mechanical parts such as, but not limited to, kick areas, infeed rollers, flatteners, straighteners, wire feeders, and progressive dies on Headers #50 (A97-2), #44 (A97-1), #41 (A96-1), and Jern Yao.  c) On or about April 12, 2022, and July 14, 2022, the employer did not ensure that operators assigned to hand-feed materials into unguarded points-of-operation were protected from the in-going nip points of rotating abrasive wheels likely to result in serious injury on Centerless grinders, Cincinnati #2 G07-16.

29 CFR  1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed.  On or about April 12, 2022 the employer did not ensure periodic inspections of energy control procedures were being conducted at least annually to ensure the requirements of the standard were being followed.  The employer did not ensure an inspection of the energy control procedures was conducted at least annually on Roll Thread Machines, Large Auto Thread Machines, Smart Roller Machines, and Headers. Hazardous energies present on the equipment include but are not limited to pneumatic, mechanical, electrical, hydraulic, and gravitational.  The Ferry Cap and Screw Company was previously cited for a violation of this Occupational Safety and Health Standard, 29 CFR  1910.147(c)(6)(i) which was contained in OSHA inspection number 1325002, citation number 1, item number 2, and was affirmed as a final order on January 18, 2019 with respect to a workplace located at 13300 Bramley Ave. Lakewood, OH 44107.
