29 CFR 1910.151(c): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use.  a) On or about March 5, 2025, employees using an acid booth for the quality control of bolts in the hand roll department were exposed to Hydrochloric Acid 20 Degree (PH is < 2) with no suitable facilities for quick drenching or flushing of the eyes and body. The employer did not ensure employees exposed to injurious corrosive materials while using the acid booth were provided with suitable facilities for quick drenching or flushing of the eyes and body.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section.  a) On or about March 5, 2025, employees adjusting the end stops, approximately 3-inches away from the grinding wheels, with an T-handled Allen key, in the TGS 1, 2, and 3 centerless grinding machines, in the grinding department were exposed to a struck-by and/or caught-in hazard from the grinding wheel. The employer did not ensure that the developed procedures were utilized for the control of potentially hazardous energy by ensuring all interlocks were functioning.  The Ferry Cap & Set Screw Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(4)(i), which was contained in OSHA inspection number 1586564, citation number 1, item number 1 and was affirmed as a final order on May 15, 2023, with respect to a workplace located at 13300 Bramley Ave, Lakewood, Ohio 44107.

29 CFR 1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed.  a) On or about March 5, 2025, employees adjusting the end stops, approximately 3-inches away from the grinding wheels, with an T-handled Allen key, in the TGS 1, 2, and 3 centerless grinding machines, in the grinding department were exposed to a struck-by and/or caught-in hazard from the grinding wheel. The employer did not ensure periodic inspections of energy control procedures for this equipment were being conducted at least annually to ensure the requirements of the standard were being followed. The employer did not provide a periodic inspection document to OSHA.  The Ferry Cap & Set Screw Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(6)(i), which was contained in OSHA inspection number 1586564, citation number 3, item number 1 and was affirmed as a final order on May 15, 2023, with respect to a workplace located at 13300 Bramley Ave, Lakewood, Ohio 44107.

29 CFR 1910.157(c)(1): The employer shall provide portable fire extinguishers and shall mount, locate and identify them so that they are readily accessible to employees without subjecting the employees to possible injury.  a) On or about March 5, 2025, employees working in the north/south corridor near the men's locker room and packaging department were not provided with readily accessible, mounted and identified portable fire extinguishers. The employer did not ensure that all fire extinguishers were mounted and identified to make them readily accessible to employees without subjecting employees to possible injuries.
