29 CFR 1926.416(a)(1): Employee(s) were permitted to work in proximity to electric power circuits that were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: On March 31, 2022, in the ceiling above Vestibule A, the power to the motion sensor power pack was not de-energized before an employee was assigned to troubleshoot the motion sensor lighting system. Employee was exposed to electrical hazards.

29 CFR 1904.39(a)(1): Within eight (8) hours after the death of any employee as a result of a work-related incident, the employer did not report the fatality to the Occupational Safety and Health Administration (OSHA), U.S. Department of Labor. On or about March 31, 2022, an employee was electrocuted and fell from a 6-foot ladder and was not reported until April 1, 2022, approximately 22 hours after learning of the fatality .
