29 CFR 1910.147(c)(7)(iii)(B): Additional retraining was not conducted whenever a periodic inspection under paragraph (c)(6) of this section revealed, or whenever the employer had reason to believe, that there were deviations from or inadequacies in the employee's knowledge or use of the energy control procedures: The AVD is amended to: On and before April 1, 2022, the employer did not provide additional retraining when it had reason to believe that there were deviations from or inadequacies in the employee's knowledge or use of the employer's energy control procedures. Abatement Date is amended to: 10/28/2022 29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a. Aurora Organic Dairy Corp. dba AURORA ORGANIC DAIRY. at 6788 State Highway 66, Platteville Colorado 80651: On and before April 1, 2022 machine specific, step-by-step procedures were not utilized for the control of potentially hazardous energy when employees were engaged in maintenance of the Westfalia Shift Unit. On March 26, 2022 an employee was injured while adjusting a pallet of product without first rendering the machine inoperable. This condition exposed employees head injuries, amputations and broken bones.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a. Aurora Organic Dairy Corp. dba AURORA ORGANIC DAIRY., at 6788 State Highway 66, Platteville Colorado 80651: On and before April 1, 2022 the employer did not adequately protect employees from point of operation hazards. Employees operated and maintained the Westfalia Shift Unit conveyor without guards to prevent injury. An employee was injured while adjusting a pallet of product. This condition exposed employees to lacerations and amputation hazards.
