OSH ACT of 1970 Section (5)(a)(1) The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and caught between/crushing hazards during crane operations due to nearby objects: Among other methods, feasible and acceptable means of abatement include, but are not limited to: 1. Perform a risk assessment/hazard analysis on storage of product/materials and the impact of storage configurations on crane operations. 2. Retrain employees on "Line of Fire" body positions, load handling and pinch points to crane operations. 3. Identify staging areas for product and/or storage configurations that will prevent struck-by/caught-between incidents with cranes. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR 1910.184(c)(8): Load(s) suspended by sling(s) were not kept clear of obstructions: a) Ellwood City Forge Company, Machine shop lay down area/Mezzanine area, on or about April 4th, 2022 and times prior thereto: An employee sustained a leg fracture when a steel block that was being hoisted with the Zenar brand crane (Serial #8607) came into contact with a round bar in the lay down area. The round bar rolled off of the "4 by 4" wooden blocks, where it was stored, onto the employees left leg during the unloading operation. The steel block was being lifted with two metal chain slings. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR 1910.176(b): Storage of material created a hazard: a) Ellwood City Forge Company, Machine shop lay down area/Mezzanine area, on or about April 4th, 2022 and times prior thereto: The storage of material created a struck-by hazard in the Machine shop mezzanine area in that, a round bar was stored too close to steel blocks that were being loaded/unloaded in the area with the Zenar brand crane (Serial #8607). An employee sustained a left leg fracture when a round bar was struck during the unloading process of a steel block in the mezzanine area. The round bar rolled onto the employees left leg during the incident. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
