The rigging's tag was missing the rated capacity.   Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

OSH ACT of 1970 Section (5)(a)(1): Section 5(a)(1) of the Occupational Safety and Health Act: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to caught between/crushing hazards associated with the below the hook lifting magnet:    (a) Ellwood City Forging Company; Fluid end cell area- on 04/09/2022, the lifting magnet (model # EPL 197) attached to the Zenar 20-ton crane released from a piece of scrap steel while attempting a lift, in a manner which caused the magnet to pinch the employees' hand between the scrap steel and magnet, resulting in a partial finger amputation. Employees were operating the magnet in a manner in which it was not designed for by the manufacturer. The lifting magnet was exposed to a shear force caused by the magnet being utilized for side pulls in that the magnet was placed on the side of the scrap steel.    (b) Ellwood City Forging Company; Fluid end cell area- on 04/09/2022 and at times prior thereto: Warning labels on side of magnet (model # EPL 197) were defaced.   ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation.  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records.
