29 CFR 1926.1053(b)(1): Where a portable ladder was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and a grasping device was not provided to assist employees in mounting and dismounting the ladder: a) Job Site - Employees used a 24 ft. Werner portable extension ladder to access the roof and the side rails extended approximately 1 foot above the landing surface. The ladder was secured at its top but no grasping device was provided to assist employees in mounting and dismounting the ladder, thereby exposing employees to an approximate 20 foot fall hazard to the ground below, on or about April 6, 2022. No abatement certification or documentation required.
