1926.20(b)(2) The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s): a) Wind Creek Hotel, top floor - On or about April 5th, 2022, an employee setting deck forms utilized a Personal Fall Arrest System rigged in a manner that would allow him to freefall more than 6 feet or contact a lower level, exposing him to serious injury in the event of a fall.
