29 CFR 1926.651(b)(3): When excavation operations approached the estimated location of underground installations, the exact location of the installations was not determined by safe and acceptable means: (a) Finley LLC. at 4700 E Evans Ave in Denver, CO: On or about April 5, 2022, the company did not use safe and acceptable means to determine the exact location of a gas line when digging a shallow trench near the line with a mini excavator. The excavator bucket contacted the line during digging, which could result in a sudden release of natural gas. This exposed employees to fire and explosion hazards.
